Belen Consolidated Schools along with UNM College of Nursing - Family Nurse Practitioner Program will host a free Sports Physical Clinic for all students participating in sports, ROTC, Band and any other activities requiring a physical to participate. The Clinic will be held on Thursday June 8th at Belen High School from 8:00 AM to 3:00 PM on a first come first served basis. Parents do not have to be present as long as students have the completed Health History and a signed form. The Sports Physical Forms can be found on the Athletics web page or athletics office. Any other required documents will need to be uploaded into Rank One prior to the physical. Contact Athletics with any questions regarding Rank One.
