A new physical, dated after April 1, 2017, is required to participate in athletics for the 2017-2018 school year. Students must also have have medical insurance to participate.
Copies of physical and insurance forms can be downloaded from the Athletic Trainer folder under Department Files on this page.
Sports Physical Form
On-time, daily attendance is mandatory for students to benefit
to the fullest from the instructional program. The district
recognizes, however, that there are situations that dictate a
student’s absence. Therefore, in accordance with district
guidelines, absences may be excused for the following reasons:
1) Doctor’s appointment
2) Death in the family
3) Religious commitment

- There are several forms available to parents that pertain to student demographics and other information. In an effort to observe FERPA laws, these forms are not placed on the web; rather, please see the site principal, or contact the Health Services Office and Curriculum office for a copy or copies.
